가 어려워 망막열공, 각막 및 홍채손상 등의 합병증이 발생할 수 있다는 문제점이 있다. 이와 같은 문제점을 해결하기 위해 전신마취하에 레이저 시술을 하려는 여러 시도가 있었다. 1980년대에 전신 마취하에 측와위 자세(lateral decubitus position)로 머리를 고정하여 레이저를 시행한 사례가 있었으며, 이러한 방법은 기존의 누워서 또는 앉은 자세에서 시행하는 레이저에 비해 필요한 장비나 환자에게 가는 부담이 적었다<ref>Kaufman LM.
