안와 결핵

안와 결핵 (ocular tuberculosis) 의 경우 폐외 결핵의 드문 형태이다.

전파 경로

• 혈액으로 이차 전파
• 코곁굴로부터의 직접 전파

분류

• Classical periostitis
• Orbital soft tissue tuberculoma or cold abscess, with no bony destruction
• Orbital TB with evidence of bony involvement, not categorized as (i)
• Orbital spread from the paranasal sinuses
• Dacryoadenitis

진단

안와결핵의 진단은 임상적으로 하게되며，보통 Mycobacterium tuberculosis가 배양되고 결핵 치료에 반응이 있다면 안와 결핵으로 진단할 수 있다. 조직검사에서 상피모양의 육아종과 함께 랑게르한스 거대세포 (Langhans giant cell) 와 치즈괴사 (caseous necrosis) 는 결핵의 전형적 소견이다. 항산막대균(acid-fost bacillus，AFB)의 확인 및 면역 화학염색을 통해 진단 특이도를 높일 수 있다.
